IntraCare and MySeema Partner to Enhance Value-Based Care Solutions

This initiative targets effective service for IntraCare’s burgeoning Medicare Advantage and Medicare Shared Savings Program (MSSP) beneficiaries by employing cutting-edge technologies, proactive care coordination, and continuous care management. Special attention is directed towards Chronic Special Needs Plan (C-SNP) members, aiming to optimize quality care across comprehensive CMS programs for intricate medical conditions.
