Humana Expands Cardiac Care Services for Medicare Advantage Members
Humana Inc. has announced an expansion of its cardiac care services for Medicare Advantage members, emphasizing value-based care models. This initiative builds upon Humana's existing collaboration with CVAUSA's Novocardia Care Solutions. New partnerships with Karoo Health, US Heart and Vascular, and Chamber Cardio will broaden the scope of Humana's cardiac care program. Members of Humana's Individual and Group Medicare Advantage plans will now benefit from this expanded access through affiliated cardiologists, enhancing quality of life and reducing hospitalizations.
The program's core elements focus on delivering value-based care through highly coordinated, evidence-driven healthcare teams. A 24/7 hotline will provide continuous access to a coordinated care team, facilitating effective management of heart conditions. Advanced remote monitoring technology allows local teams to track key indicators such as heart rate, rhythm, and blood pressure, ensuring timely interventions when necessary. Additionally, an integrated care support system standardizes communication and synchronization between care managers and cardiologists, aligning medical tests, medications, and patient-specific dietary recommendations.
According to Humana's analysis, implementing value-based care significantly improves healthcare quality and adheres to evidence-based practices for cardiac patients. The expanded access includes services from CVAUSA's Novocardia in states like Florida, Georgia, and Illinois, Karoo Health in Arizona, Alabama, and Louisiana, Chamber Cardio in Maryland and Texas, and US Heart and Vascular across Alabama, Louisiana, and Kansas. Additional providers are available through Humana's network, with opportunities for further contracting with other plan sponsors.
