Clover Health Launches AI-Powered Community Pharmacy Pilot in New Jersey
Clover Health Investments, a Medicare Advantage provider, has launched a community-based pharmacy pilot program in New Jersey in partnership with IPC Digital Health, a division of Independent Pharmacy Cooperative. The initiative leverages IPC’s iCare+ Independent Pharmacy Network to support seniors in managing their medications and maintaining health at home through local pharmacies.
These pharmacies utilize Clover Health’s AI-driven tools and real-time data to monitor prescription adherence, identify gaps, and coordinate care with physicians and Clover’s care teams. The program aims to enhance medication management for seniors with chronic conditions, thereby reducing avoidable hospital visits and improving healthcare outcomes within local communities.
