Medicare Outpatient Payment System Update for 2027

The Centers for Medicare & Medicaid Services (CMS) has announced a 2.3% net increase in the Medicare Hospital Outpatient Prospective Payment System (OPPS) rates for 2027, reflecting broad changes that stand to impact hospitals significantly, especially those in rural and underserved areas.

This adjustment comes with a 3.2% increase due to the market basket update, partially offset by a 0.9 percentage point productivity reduction. CMS has also expanded its site-neutral payment policy, including imaging services without contrast at certain outpatient departments. These rates are set at 40% of the standard OPPS rate, which CMS projects will reduce outpatient PPS spending by $260 million in 2027. However, rural sole community hospitals are exempt from these changes, emphasizing CMS's attempt to balance cost savings with vital service access.

Industry Challenges and Concerns

Ashley Thompson, from the American Hospital Association (AHA), has been vocal about the incongruity of CMS's outpatient payment rates with the financial hurdles many hospitals face, particularly in rural areas. Concerns are mounting about the new site-neutral payment policies and their potential to exacerbate financial difficulties. Thompson noted that outpatient departments of hospitals typically handle more complex patient needs, making the new payment policy particularly challenging.

340B Drug Pricing Adjustments

CMS also announced new 340B drug pricing rates, set at the average sales price minus 28%. This marks a shift from the initially proposed ASP minus 33.4%, and will be implemented in a budget-neutral manner. This change will result in a 7.5% payment increase for non-drug items and services. While certain hospitals are exempt, regular non-340B hospitals will continue to receive payments at ASP plus 6%. The planned rapid recoupment of prior 340B adjustments will see a PPS conversion-factor reduction increase, raising concerns about long-term financial impacts on hospitals and their ability to service the 340B program.

Additional Regulatory Developments

Among other changes, CMS plans to remove 637 services from the inpatient-only list, expand compliance verification responsibilities for accrediting organizations, and adjust prior authorization requirements, which now include botulinum toxin injection codes starting July 2027. Additionally, hospitals must secure separate national provider identifiers for each off-campus outpatient department by January 2028.

Change Impact
OPPS Rate Increase 2.3% net increase for 2027
Site-Neutral Policy PPS spending reduced by $260 million
340B Drug Pricing Average sales price minus 28%
Recoupment $7.8 billion by 2031

These regulatory updates, as outlined by CMS, are poised to redefine operational and financial landscapes for many hospitals across the U.S. Insurance professionals, particularly those working within hospital networks, will need to carefully navigate these changes to effectively manage both compliance and operational strategies.
