Qualifying for the Medicare Grocery Card
Eligibility for the Medicare Grocery Card is linked to qualification for a C-SNP, which serves individuals with specific chronic health conditions. The Centers for Medicare & Medicaid Services (CMS) has identified 15 qualifying conditions, including chronic cardiovascular disorders, diabetes, and cancer. Enrollment involves filling out a Verification of Chronic Condition (VCC) form, signed by a physician, which must be completed during the annual open enrollment period starting on October 15.

The popularity of C-SNP plans is on the rise, with recent enrollment figures surpassing 1.1 million according to Stidom. However, it's worth noting that fewer than 5% of Medicare enrollees are currently in a C-SNP, despite 68% of all Medicare beneficiaries having two or more chronic conditions. The card is accepted at well-known retailers like Kroger, Publix, Safeway, and Walgreens, with preloaded amounts typically ranging from $25 to $200.
