Elderplan HomeFirst Opens New Office in Brooklyn to Enhance Medicare Access

Elderplan/HomeFirst, a nonprofit leader in senior healthcare management, has inaugurated its inaugural office in Brooklyn at 6405 Seventh Avenue, Sunset Park. This new branch caters to Medicare Advantage plan members, aiming to enhance their understanding of coverage, access to benefits, and participation in health and wellness initiatives. The facility also serves potential beneficiaries seeking information and enrolling in health plans.

Founded in 1985, Elderplan was a pioneer in Medicare Social HMO demonstration projects within the United States. It offers comprehensive coverage for Medicare and dual Medicare/Medicaid beneficiaries, addressing a spectrum of health needs with specialized plans for individuals eligible for nursing home care but residing at home. These offerings extend to diverse care community residents, promoting comprehensive care solutions.

HomeFirst began in 2001 as a Managed Long-Term Care Plan in New York State, focused on supporting members needing daily living assistance for home safety. The program proudly achieved a 5-star rating in the New York State Department of Health's consumer guide. David Wagner, CEO of MJHS Health System & Elderplan/HomeFirst, emphasized the significance of expanding into Brooklyn, aligning with their commitment to advancing healthcare support for members. This Brooklyn office complements other locations in Flushing and Washington Heights, reinforcing their mission to broaden healthcare accessibility.

State Senator Stephen Chan and Council Member Susan Zhuang endorsed the new office, acknowledging the upgraded healthcare resources for their community. Zhuang, chair of the Council’s Committee on Aging, highlighted the importance of a location in a neighborhood frequented by seniors and caregivers. The Brooklyn office is open weekdays from 9:00 a.m. to 5:00 p.m., with weekend appointments available via their website or phone.
