Medicaid Reform: Addressing Expenditure Growth and Protecting Taxpayers

The National Taxpayers Union has sent a letter to Congressional leaders, advocating for Medicaid reform to ensure fiscal responsibility and support for vulnerable populations. The letter highlights significant program spending and structural issues, stressing the importance of maintaining Medicaid's mission while safeguarding taxpayer resources. Between 2014 and 2024, Medicaid expenditures increased by nearly 88%, marking it as one of the fastest-growing federal programs.

The surge in Medicaid spending is primarily attributed to the Affordable Care Act's expansion, which increased enrollment by offering higher federal matching rates for states. This broad enrollment shift has diverted resources away from vulnerable groups, such as low-income children. Additionally, the entitlement nature of Medicaid, with automatic federal matching funds, has led to substantial improper payments. In fiscal year 2025, these payments were estimated at over $37 billion. The Union suggests that since states do not fully bear Medicaid costs, they lack strong incentives to prevent fraud and eligibility errors.

Furthermore, the Union criticizes states' widespread use of provider taxes to enhance Medicaid funding, which disproportionately burdens the federal government, now covering about two-thirds of spending. To enhance Medicaid's sustainability amid federal debt challenges, recent reforms have been introduced, such as the Working Families Tax Cuts. These include capping state-directed payments and introducing work requirements for some beneficiaries. The Union recommends expanding such reforms and exploring funding strategies that boost state involvement, promoting rigorous oversight to reduce waste and safeguard Medicaid's future viability.
