HarmonyCares ACO LLC, headquartered in Troy, Michigan, has garnered recognition as the second-best performing participant in the ACO REACH program, as disclosed by the Centers for Medicare & Medicaid Services (CMS). In 2024, the company achieved a commendable net savings rate of 25.8% in the High Needs Track of ACO REACH, ensuring high-quality care for Medicare's most at-risk patient groups. Among 115 participating accountable care organizations (ACOs), HarmonyCares showcased a robust alignment of quality and cost-effectiveness, addressing the needs of some of the highest-need, highest-cost patient demographics.

The ACO REACH model, initiated by CMS's Innovation Center, allows participants to take on financial risk in managing spending and care quality for specified populations, sharing in the savings if benchmarks are met. In 2024, the model generated $2.5 billion in gross savings, with nearly $1 billion returned to Medicare, covering 2.5 million beneficiaries.
The ACO REACH initiative is set to conclude in 2026, paving the way for CMS's new LEAD Model in 2027. This upcoming framework will integrate High Needs policies for all qualifying beneficiaries within ACOs, continuing the focus on complex Medicare patient care.
