Medicare Pilot Program Enhances Service Approval Process with AI
Medicare has launched a pilot program in six states, including Oklahoma, to test preapproval requirements for specific medical services through prior authorization. The initiative leverages artificial intelligence to enhance the approval process, aiming to curb fraudulent claims and ensure the provision of essential care. Despite its intentions, the Wasteful and Inappropriate Service Reduction Model (WISeR) has faced implementation hurdles, resulting in delays and confusion among healthcare professionals and patients.
The pilot states—Arizona, New Jersey, Ohio, Texas, Washington, and Oklahoma—kicked off the program in January. The focus is on services prone to misuse, like epidurals and spinal surgeries. Feedback from these regions suggests the rollout was hasty. Todd Baker, former CEO of the Ohio State Medical Association, pointed out that healthcare providers had to rapidly adapt to new procedures without adequate preparation.
Government contractors acknowledge the swift implementation. Jeremy Friese, CEO of Humata Health, noted ongoing technological refinements post-launch. The program promises a 72-hour decision turnaround, achieving an 88% approval rate for compliant cases. However, substantial delays have been reported, with institutions like the University of Washington experiencing backlogs for epidural injections.
Medicare's approach seeks to restrict inappropriate treatments without hindering necessary care. Abe Sutton from the Center for Medicare and Medicaid Innovation emphasized refining the approval process without disrupting care. Mehmet Oz from CMS stressed that prior authorization is vital for safeguarding appropriate procedures. Despite unresolved issues, discussions on potential expansion persist.
Mary Clarke, a family practice physician, suggests success in this pilot could lead to broader implementation. Although CMS hasn't confirmed plans beyond current services, it continues to evaluate the program's impact on Medicare appeals and costs. The role of AI in procedural approvals has raised concerns, as some suspect that inaccuracies could lead to unjustified denials, affecting expenditure and efficiency.
