The American Hospital Association (AHA), representing nearly 5,000 healthcare entities and numerous professionals, recently commented on proposed regulatory changes to federal financial assistance. These changes, suggested by the Office of Management and Budget (OMB) alongside other agencies, raise significant concerns for the healthcare industry. The AHA is particularly apprehensive about modifications to the Uniform Grants Regulation that could allow agencies to alter federal award conditions or terminate funding based on agency priorities without a statutory mandate.
This proposal could grant agencies the power to end grant funding unilaterally, broadening termination reasons and potentially eliminating appeals processes. Such developments introduce uncertainty for healthcare providers that rely on stable federal support for long-term planning, medical research, workforce education, and healthcare infrastructure investments.
Hospitals and health systems depend heavily on federal grants to support initiatives like emergency preparedness, maternal and rural health, and public health projects. These initiatives require substantial investment in staff, technology, and community collaborations, all premised on consistent grant funding. Any uncertainty regarding grant stability threatens to disrupt these critical services and advancements.
While the AHA acknowledges efforts to enhance fiscal accountability, it warns that the proposed changes could deter healthcare providers, especially those most vulnerable, from participating in grant programs due to increased financial unpredictability. The AHA recommends that OMB ensure grant stability by mandating conditions for changes or terminations, such as noncompliance or legislative alterations. Additionally, the AHA calls for clear guidance on unaffected Medicare and Medicaid payment streams, highlighting programs like the Medicare Rural Hospital Flexibility Program. Maintaining predictability in grant allocations is critical for enabling healthcare institutions to make investments that benefit patients and communities, as originally intended by federal assistance programs. The AHA seeks ongoing collaboration with OMB to balance accountability with the need for stable funding.
