Medicare Expands Access to GLP-1 Weight Loss Medications
Individuals with Medicare or Medicare Advantage can now access GLP-1 weight loss medications at a monthly rate of $50, thanks to a new initiative aimed at expanding eligibility. This effort seeks to clarify eligibility and usage for first-time users, according to Dr. Josh Neal and Nicole Martinez de Andino from MUSC Health.
Dr. Neal, an obesity medicine physician, notes that the program primarily benefits those with Medicare Part D, which covers drug aspects of Medicare plans, including most Medicare Advantage plans. Eligibility is based on specific BMI criteria and the absence of exclusions related to existing coverage for GLP-1 drugs for other conditions.
Exclusions and Eligibility Criteria
Key exclusions involve coverage for GLP-1 medications for conditions like diabetes or metabolic dysfunction-associated liver disease. Qualified individuals typically include those with a BMI over 35, or a BMI over 30 with specific health conditions such as heart failure or chronic kidney disease. Additionally, a BMI of 27 coupled with conditions like pre-diabetes or cardiovascular events may meet the standards.
Nicole Martinez de Andino, a nurse practitioner and registered dietitian, explains that GLP-1 medications aid weight management by slowing gastric emptying and reducing food-related thoughts. This assists in making healthier eating choices, although common side effects like nausea and constipation may occur due to delayed stomach emptying.
Clinical trials indicate a weight reduction of 15 to 20%, though real-world results may be lower. Neal emphasizes the importance of integrating lifestyle changes with medication, supported by digital health coaching initiatives from MUSC, focusing on nutrition and physical activity.
The MUSC Weight Management Center has expanded services to support increasing GLP-1 medication usage. Resources include virtual classes and expert dietary, exercise, and behavioral guidance. Programs are designed to promote nutritional balance, muscle-preserving exercises, and lasting lifestyle changes.
The Medicare GLP-1 Bridge initiative, available until December 31, 2027, offers injectable options like Wegovy and oral forms including Wegovy pills at program rates. This temporary initiative precedes the potential BALANCE program, aiming to negotiate favorable drug prices, thus ensuring sustainable solutions beyond the Bridge's timeline.
