Innovative Strategies in Sleep Apnea Management for Medicare Patients

Approximately 30 million individuals in the United States are affected by obstructive sleep apnea (OSA), with an estimated 80% remaining undiagnosed. This condition often goes unrecognized in those over 65, as symptoms like fatigue, headaches, and disrupted sleep are frequently considered normal aspects of aging. Gender differences in symptom presentation and assumptions that tiredness results from a busy lifestyle further contribute to underdiagnosis.

OSA significantly impacts health, linked to increased cardiovascular risks and complications such as hypertension and heart failure. It is also associated with higher healthcare costs due to emergency visits and hospitalizations. Medicare Advantage plans are focusing on closing these care gaps, as early detection and effective management are incentivized through Star ratings and chronic disease programs.

Essence Healthcare, a Medicare Advantage provider, is employing innovative strategies to tackle these challenges. Beginning in the 2025 benefit year, they introduced the Oura Ring and Oura Membership as supplemental benefits for members. This initiative aims to leverage member-generated data for early health risk identification, particularly focusing on sleep apnea through metrics provided by Oura's sleep features.

Innovations in Sleep Apnea Management

"Sleep apnea is prevalent and often overlooked, affecting cardiovascular health and quality of life," stated Dr. Saria Saccocio, Essence Healthcare's Chief Medical Officer. With Oura insights, early symptom identification can prompt timely referrals back to physicians. Traditional detection methods, which often rely on in-clinic assessments, are time-intensive and only occur when symptoms become severe, leading to accumulated health risks.

Essence Healthcare's program addresses these challenges by collaborating with various entities to ensure members are screened using wearable data, followed by proper follow-ups with doctors. Members participating in the program share their Oura data, which signals potential breathing disturbances, monitored via the Breathing Disturbance Index as a wellness alert, not a diagnostic tool.

Closing Care Gaps Through Technology

The program aims to reach all identified members, ensuring proper clinical evaluation. Trust-building communications, such as pre-call text messages, precede AI-assisted outreach efforts. Members are guided through the STOP-BANG questionnaire to assess the likelihood of OSA, with positive results communicated to primary care providers for further follow-up.

Proactive Outreach and Improved Outcomes

Essence's approach demonstrates how integrating technology and proactive outreach can close care gaps and improve health outcomes. By systematically using wearable data, AI-driven outreach, and coordinated clinical handoffs, they address high-priority health conditions beyond sleep apnea, such as cardiometabolic risk and fall prevention. "This program highlights the potential of coordinated efforts among health plans, clinicians, and technology partners to support member health proactively," Dr. Saccocio added, underscoring the importance of physicians in care decisions and setting a model for future personalized care strategies.
