Trends in Medicare Reimbursement for Eyelid Procedures

The Centers for Medicare & Medicaid Services (CMS) have demonstrated varying trends in reimbursement rates for two related eyelid procedures. Notably, Medicare reimbursements for eyelid abscess blepharotomy (CPT code 67700) have increased beyond inflation, whereas those for chalazion incision and drainage (CPT code 67800) have diminished. This divergence highlights significant shifts in reimbursement priorities that could impact provider payment strategies.

In a comprehensive study by Jaber J, Siddiqui T, and Onyeka R, data spanning from 2000 to 2023 was scrutinized using CMS's Physician Fee Schedule Look-Up Tool. Adjustments for inflation were made employing the Consumer Price Index from the U.S. Bureau of Labor Statistics. The results uncovered an inflation-adjusted rise in reimbursements for eyelid abscess blepharotomy by 9.23% for facility fees and 11.76% for nonfacility fees. Conversely, chalazion incision and drainage reimbursements decreased markedly by 20.48% for facility fees and 57.95% for nonfacility fees in the same period.

Evaluating Discrepancies in Ophthalmic Procedure Reimbursements

The study proposes that these discrepancies could be linked to the higher overall relative value unit (RVU) and lower frequency of the blepharotomy procedure compared to the more common chalazion incision. However, the omission of private insurance data limits the scope, suggesting these findings may not entirely represent trends across the broader payer landscape. Ophthalmologists should be mindful of these Medicare and Medicaid reimbursement reductions, as they may influence discussions with policymakers.

Additionally, related CPT codes such as 67801, 67805, and 67808 were not evaluated, indicating potential avenues for further exploration.
