ALYKA Health Joins CMS ACCESS Model to Enhance Medicare Outcomes

ALYKA Health, a digital health company specializing in patient engagement and cardiovascular disease prevention, has announced its participation in the Centers for Medicare & Medicaid Services (CMS) ACCESS Model. This national initiative, known as Advancing Chronic Care with Effective Scalable Solutions (ACCESS), seeks to enhance care management and outcomes for Medicare beneficiaries with chronic conditions.

The ACCESS Model, part of CMS's Center for Medicare and Medicaid Innovation, aims to improve healthcare delivery by aligning payment with outcomes, encouraging the use of technology to manage and prevent chronic diseases. ALYKA Health was chosen after a detailed selection process and will employ its technology platform to provide coordinated, personalized care to Medicare patients, aligning with CMS’s strategy to transition healthcare towards value-based care models.

Dr. Ali Rahimi, ALYKA Health's CEO, commented, "This opportunity validates our digital-first approach to care engagement and allows us to showcase how technology can address care gaps for Medicare beneficiaries." Under the initiative, ALYKA Health will focus on conditions such as obesity, hypertension, high cholesterol, coronary artery disease, congestive heart failure, atrial fibrillation, and chronic kidney disease. The platform provides real-time health data, automates care gap identification, and offers personalized engagement tools, aiming to reduce preventable hospitalizations and improve patient quality of life.

Focus on Improved Outcomes

The ACCESS Model encourages healthcare organizations to provide better-coordinated care for beneficiaries with multiple chronic diseases, rewarding improvements in healthcare outcomes and cost reductions. It covers four condition categories, namely cardio-kidney-metabolic conditions in early and later stages, musculoskeletal, and behavioral health conditions. ALYKA Health is involved in managing two of these categories and must comply with specified standards, including Medicare Part B enrollment, data privacy, security, and quality reporting. Major private insurers have also agreed to align with the payment model, which started earlier this year.

Dr. Amit Thosani, Chief Medical Officer, stated, "The ACCESS Model provides structural and financial incentives for large-scale improvements in patient engagement and care," emphasizing ALYKA Health’s commitment to enhancing Medicare systems. The company will commence participation on July 5, 2026, and intends to report model performance outcomes alongside CMS, utilizing the insights gained to refine its solutions and contribute to value-based care advancements.

ALYKA Health’s engagement platform is designed to enhance health literacy and accessibility, integrating real-time data and evidence-based content to keep patients informed and engaged between medical visits. By partnering with health systems and care providers, ALYKA Health aims to promote self-management of chronic diseases and reduce preventable heart conditions, leveraging its unique combination of AI personalization and efficient health system integration.
