Improving Healthcare Funding Efficiency Through Digital Infrastructure

Each year, substantial funds allocated to healthcare in low- and middle-income countries experience significant delays or fail to reach their intended destinations. This issue arises not from a lack of funding but due to the inefficiencies embedded in traditional financial systems. Digital public infrastructure provides a viable solution to these challenges. A report by CHAI and the eGovernments Foundation outlines strategic recommendations to alleviate these issues and proposes actionable steps for improvement.

The global health sector is grappling with the challenge of maximizing outcomes amidst limited resources, intensified by reductions in foreign aid and fiscal constraints. Alongside financial pressure, there is a critical emphasis on ensuring that available funds are efficiently channeled to healthcare facilities.

Illustratively, a health worker in Burkina Faso experienced a six-month wage delay due to flawed identity records and manual verification processes that impeded payment confirmations. Similarly, $3.25 million in medicines expired in Eswatini's warehouses while local health facilities faced shortages, highlighting the inefficiencies of disconnected stock visibility systems. In Ethiopia, a donor fund struggles to utilize its budget fully due to restricted procurement insights and fragmented reporting systems.

World Bank data from 64 low- and middle-income countries reveals that approximately 15 percent of government health allocations remain unspent. This issue is even more pronounced with donor contributions, where underspending aligns with primary healthcare allocations in some low-income African countries. Inefficient financial systems and fractured infrastructures contribute significantly to this underspending.

As international donors express interest in channeling resources directly through local systems, the efficiency and transparency of these systems become critical. Rather than undertaking prolonged technology assessments, digitizing transactions at their origin and employing standard data practices can enhance accountability and transparency without overhauling existing frameworks.

Historically, governments have leaned on Integrated Financial Management Information Systems and Enterprise Resource Planning for comprehensive data management. Although these systems show promise, they were primarily designed for central treasury operations, thus lacking the granularity required for effective frontline health service delivery.

This challenge revolves around standards. Each governing entity or donor operates on its system classifications, hindering data exchange and effective communication. The solution is a shared digital infrastructure offering a universal platform for existing systems to collaborate seamlessly. Several nations have already seen improvements from adopting this approach, such as in India, where integrating data verification processes significantly reduced wage payout times without replacing existing financial systems.
