Several prominent healthcare organizations in Upstate New York—Community Care Physicians (CCP), Albany Med Health System, and CDPHP—have launched a collaborative endeavor aimed at revolutionizing healthcare delivery in the Capital Region. Despite each entity operating independently, the alliance is committed to improving both access to care and patient outcomes. In 2021, CDPHP and CCP initiated a strategic partnership to consolidate services at the Wellness Way location. This integration facilitates a comprehensive care environment where patients can receive medical services, perform lab tests, access diagnostic screenings, pick up prescriptions, and interact with CDPHP representatives. The recent inclusion of Albany Med Health System expands this partnership further. As the only academic health system in the area, Albany Med aims to seamlessly integrate physician practices with hospital and health plan operations while preserving patient autonomy. The collaboration focuses on strategic initiatives such as guiding patients to appropriate care facilities, refining referral processes from CCP to Albany Med Health System, and enhancing information exchange with CDPHP and healthcare providers to elevate patient care quality. With these objectives, Albany Med Health System has launched the Albany Med Rheumatology & Infusion Center at Wellness Way. This facility is poised to extend Albany Med's rheumatology services in Latham, supported by a fellowship program that guarantees a continuous influx of trained specialists to meet community demand. Dr. Martin Stallone, Executive Vice President and Chief Healthcare Services Officer at CDPHP, affirmed the benefits of this partnership, stating, “This innovative arrangement represents a pivotal advance in delivering patient-centered care by leveraging the strengths of CDPHP, CCP, and Albany Med Health System.” Dr. Ronald Musto, CEO of Community Care Physicians, emphasized the improved communication within the healthcare network, noting that the new center will support coordinated, high-quality care from esteemed local providers.
