Medicare Advantage in Upstate New York Faces Financial Strain Amid Wage Index Changes

In upstate New York, nearly 450,000 residents enrolled in Medicare Advantage (MA) are facing significant changes due to a mismatch between federal funding and regional hospital cost structures.

A recent alteration in the Medicare wage index, aimed to adjust payments to reflect regional healthcare labor costs more accurately, has inadvertently caused financial distress for Medicare Advantage plans.

The updated index increased the reimbursement rates for hospitals by 20-40% starting October 1, 2023, intended to bring $967 million more annually to struggling upstate hospitals. However, the change did not correspond with an increase in the premiums paid by the Centers for Medicare and Medicaid Services (CMS) to Medicare Advantage plans, triggering financial losses.

Prominent nonprofits like Excellus BlueCross, MVP Health Care, and CDPHP are now advocating for legislative or regulatory resolutions to prevent further financial strain which could lead to reductions in plan offerings, increased premiums, and decreased benefits for seniors.

Affected companies and representatives, including Rep. Claudia Tenney, have been proactive. Tenney has reintroduced a bipartisan bill to authorize CMS to adjust its rates in response to the wage index change.

Moreover, representatives from both parties across New York are supporting this measure, advocating for a solution to ensure these plans remain viable without reducing benefits or increasing costs to beneficiaries. This legislative action underscores the necessity to balance regional healthcare needs with the financial stability of Medicare Advantage plans, ensuring that senior citizens in upstate New York do not bear the unintended consequences of policy adjustments.
