Ohio Health Department Moves to Revoke Insight Hospital License Over Compliance Issues

Insight Health System is scheduled to meet with the Ohio Department of Health to address the state's intention to revoke the operating license of Insight Hospital & Medical Center Trumbull in Warren. The revocation is based on multiple regulatory violations identified during inspections dating back to March 27, including failure to maintain appropriate emergency room conditions and inadequate integration of emergency services with other hospital departments. A notable concern involved delays in administering critical stroke treatment medications, which endangered patient care. Insight Health System's facility has faced operational challenges throughout 2025, including two closures attributed in part to financial pressures linked to previous ownership by Steward Health Care. Although the Ohio Department of Health approved a reopening in October, CMS revoked the hospital's certification shortly prior, impacting Medicare and Medicaid reimbursement. Following the revocation notice, Insight Trumbull ceased operations later that month. The Ohio Department of Health cited longstanding uncorrected violations covering delayed maintenance, life safety, water management, patient care, and building heating issues. The department indicated it had refrained from closing the hospital earlier to provide opportunities for compliance and resumption of full services. The license revocation hearing will be private, allowing Insight to present evidence and arguments to contest or address the state's concerns.
