CMS-Funded GUIDE Program Launches to Enhance Dementia Care Coordination

The GUIDE Program (Guiding an Improved Dementia Experience) is a newly launched initiative funded by the Centers for Medicare & Medicaid Services (CMS) aimed at enhancing care for individuals with dementia and supporting their caregivers. Administered by AgeSpan in collaboration with CCS Health and Healthspan Partners, this program offers person-centered, coordinated care at no cost to eligible participants. It targets individuals diagnosed with mild to severe dementia who are enrolled in traditional Medicare Parts A & B and live in community settings such as their own homes or assisted living, excluding nursing home residents and those enrolled in Medicare Advantage or specific other programs.
